Why it matters
Segment disclosure is the primary window investors and lenders have into performance drivers of a diversified healthcare group; defining segments around an idealized industry structure instead of the CODM's actual internal reporting hides how the business is genuinely managed and breaks comparability with how the board itself makes resource-allocation decisions.
Diagnostic questions
What financial information does the chief operating decision maker actually review regularly to allocate resources and assess performance, regardless of the formal legal or reporting structure? Are components such as the hospital network, pharma manufacturing and distribution managed and reported internally as distinct components with their own results? Do any of those components independently meet the quantitative thresholds (revenue, profit/loss, assets) that would require separate segment disclosure?
Recognition
1) Identify the chief operating decision maker (CODM), the person or group (CEO, executive committee) who actually allocates resources and assesses performance, not a theoretical ideal. 2) Review the financial information the CODM regularly reviews (monthly management packs, board papers) to determine how the business is actually segmented internally (e.g. hospitals/clinics vs. pharma manufacturing vs. distribution). 3) Identify operating segments consistent with that internal structure, even if it does not align with an external industry classification. 4) Apply the quantitative thresholds (10% of combined revenue, 10% of combined profit/loss, or 10% of combined assets) to determine which segments are separately reportable, aggregating similar segments only where the aggregation criteria are met. 5) Ensure at least 75% of total external revenue is covered by reportable segments, adding more if needed. 6) Reconcile segment totals (revenue, profit/loss, assets) to consolidated financial statement totals.
القياس
Measurement
Report segment measures on the same basis used internally by the CODM for decision-making, even if that differs from IFRS measurement principles used in the primary financial statements (e.g. internal management accounts may allocate corporate overhead differently); provide a reconciliation of segment revenue, profit/loss and assets to the equivalent consolidated IFRS amounts, explaining material reconciling items.
Presentation & disclosure
Disclose the factors used to identify reportable segments and the types of products/services generating revenue in each; disclose segment revenue (external and inter-segment), profit/loss, and assets, with reconciliations to consolidated totals; disclose the measurement basis used for segment reporting if it differs from IFRS; disclose entity-wide information on revenue by major service line and geography, and major customer/payor concentration (10% or more of total revenue) even if not organized as separate segments.
Common mistakes
- Defining segments based on an idealized industry structure (e.g. by medical specialty) rather than actual internal management reporting reviewed by the CODM
- failing to reconcile segment totals to consolidated financial statements
- omitting entity-wide disclosures on customer/payor concentration when segment disclosures alone do not capture it
- applying the 10% quantitative thresholds to the wrong base (e.g. total assets instead of combined segment assets)
- aggregating dissimilar segments (different risk/return profiles) purely to reduce disclosure volume without meeting the aggregation criteria.
Simple example
A healthcare group's executive committee reviews monthly results split between 'Hospitals & Clinics' (revenue SAR 380,000,000) and 'Pharmaceutical Manufacturing' (revenue SAR 70,000,000); both exceed the 10% revenue threshold of combined revenue (SAR 450,000,000), so both are reported as separate operating segments. This is a disclosure-only exercise; there are no distinct journal entries for segment identification itself. The underlying revenue and cost transactions are recorded through normal entries (e.g. Dr Cash/Receivable / Cr Revenue for each segment's transactions), and the segment note simply re-presents already-recorded consolidated figures split by the two identified segments, reconciled to the SAR 450,000,000 total consolidated revenue.
Professional judgment
Determining the chief operating decision maker and the appropriate level of internal reporting to use as the basis for segments is judgmental, especially in matrixed organizational structures where multiple committees review different cuts of the same data; a further grey area is whether two components with similar economic characteristics (e.g. two hospital sub-networks) genuinely meet the aggregation criteria or should remain separately reported.
